RECONSTRUCTION OF SOFT TISSUE DEFECT USING THE G RACILTS MYOCUTANEOUS FLAP



Abstract
There has been much development of clinical application of myocutaneous flap and microsurgery in recent decades in the scope of plastic and reconstructive surgery. Various types of r~yocutaneous flap were chosen according to the site and extent of defect. Since Pickrell has used the gracilis muscle flap for correction of the rectal incontinence, gracilis muscle has been used for reconstruction of the vagina and ischial pressure sore as myocutaneous island flap. And in 1978, Manktelow et al had used the functioning gracilis myocutaneous free flap for active finger flexion.
The gracilis myocutaneous flap has several characteristic advantages;
I) relatively easy to elevate
2) reliable neurovascular pedicle with single easy reparable -motor nerve
3) long length of muscle for a good range of muscle excursion
4) long tendon of insertion and short fibrous origin
5) minimal donor site morbidity
And advantages of the gracilis myocutaneous island flap in reconstruction of the scrotum are:
1) one stage procedure
2) relatively thin and large flap
3) easily reach to perineum
Authors performed the operations for 2 cases of soft tissue defect on the scrotal region using the gracilis myocutaneous island flap, l case of depression deformity on cheek after reconstruction of mandible using the gracilis muscle free flap and 1 case of soft tissue defect with flexor deficiency on forearm, wrist and hand using the gracilis myocutaneous free flap with microneurovascular aiiastomosis.
The results in most instances were satisfactory and acceptable.
